I was wondering if anyone has examples of how they record information that is disccussed during appointments with doctors.
I am on a mixture of six monthly and annual check ups. When I go back in for my appointments I have trouble remebering (clearly) what information had been discussed during the previous appointment.
I am beginnig to think that I might need to start writing progress notes on myself!
Any ideas are welcome.

Hi Beck,
Have you asked your hospital for a patient diary? Many people find these useful. There are several diaries available - you may be able to get on from the Leukaemia Foundation. I know the Cancer Council NSW has a good book also.
These diaries allow you to take notes, write questions to ask your doctor, and chart your blood counts etc... Very good if you are having trouble remembering.

Hi Anto,
The patient diary is a great suggestion. I haven't come across these.
I have found a couple of resources on the Livestrong website that maybe useful but I shall definitely look into the patient diaries too.
